The RA request form is required when returning a product for repair. If you intend to drop the product off in person, an Apus technician will fill one in for you.

The completed form must be faxed to Apus for approval. A RA number will then be issued to you. This RA number must be labelled clearly on all products to be returned. Customers are to retain this number during the RA process for reference, losing it may cause unnecessary delays.
